Maximin Attendance to the Conference is 250 persons

Registration will be strictly on a first come first serve basis

   IMPORTANT - Please note that there will be two means for making payment to    the Conference, the seminar and workshops, i.e., credit card or check. Your fees ( in US Dollars) may be based on the method of payment that you select. In determining your fees please note the following notations:

     Conference = Complete admissions to all Conference sessions, trip to Edwards BOC, reception, exhibits and                              banquet at a fee of $1495.00
     Seminar = Three day Lyophilization Seminar by Dr. Thomas A. Jennings at a fee of $1995.00 (Closed)
     Seminar + Conference = $2995.00 (Closed)
     Workshop A = Lyophilizers -Commissioning and Validation ($450.00 without Conference and $400 with                               Conference.)
     Workshop B = Mimi lyophilizers reports and graphic display ($450.00 without Conference and $400 with                               Conference.) (Cancelled)
     Workshop C = Sterilization and Thermodynamic Control ($450.00 without conference and $400 with                               Conference.) (Cancelled)
     Workshop D = Maintenance of Freeze Dryers ($450.00 without Conference and $400 with Conference.)

Important: Please note that you cannot register for the seminar and a workshop. The last day of the seminar falls on the same day that the workshops are given.

FIRST MEANS: Credit cards are the preferred method of payment. We are only able to process credit cards from American Express, Master Card and Visa. If you wish to make payment by credit card at a secure web site please click on Credit Card Payment.

SECOND MEANS: You may register and make payment by check payable at a United States Bank. Please note we will not accept Bank Drafts. Those persons not living in the United States and Puerto Rico will be required to add an additional $100.00 (US Dollars) to cover the bank transfer fees

Important Registration Information - Please read before you register.

Registration: If you register by check, payment must be received on or before October 1, 2002
Accommodations: Attendees to the seminar, workshops or conference are responsible for making their own hotel reservations. For on line hotel reservations contact Radisson SAS Hotel Amsterdam Airport.
Cancellations: Refunds will be honored less ($200.00) for registration to the seminar, one or more workshops, Conference or combination thereof if written notice of the cancellation is received on or before October 4, 2002. No refund will be honored after this date. The Society reserves the right to cancel the seminar or any workshop before October 1, 2002. Attendees will receive a full refund except the $100.00 bank transfer on checks made by those residing outside the United States or Puerto Rico. Please note that that Society accepts no responsibility for any charges resulting from cancelled travel arrangements.
Transfer: You may transfer from one workshop to another if there is room and you give written notice to the Society Office by on or before October 4, 2002.
Substitutions: Substitutions will be permitted prior to the start of the seminar, workshop or Conference. No substitutions will be permitted during the course of any of these programs
No Shows: The attendees who sign up for the seminar, workshop, Conference or combination thereof and do not appear nor send a substitute will NOT receive a refund of their fees.
Waiting List: If you are on a waiting list for any of the above programs and no space becomes available, you will receive a full refund except the $100.00 bank charge if you reside outside the United States or Puerto Rico and have payed by check. We will make every effort to see that such checks are not cashed and will be returned to the sender.
Membership: To attend the Conference you MUST be a member of the Society. If you are not a member you can submit this application form but your acceptance to attend the conference will be predicated on your becoming a member prior to the start of the Conference. To become a member just complete the Membership Application.
