Important Registration Information - Please read before you register.

• Registration: Registration is not complete until payment of the fees are made to the Society. Payment must be made on or before September 5, 2003. Deadline for Seminar registration is August 29, 2005
• Accommodations: Attendees to the seminar, workshops or conference are responsible for making their own hotel reservations. Please note that in order to take advantage of the "special" room rate which is subject to change after June 14, 2003 provided you make your room reservation at this time. • Cancellations: A refund less !0% service fee will be honored for registration to the seminar, workshop, Conference or combination thereof if written notice of the cancellation is received on or before August 29, 2002. After August 29, 2003 a refund will be made less 20% of the fees. The Society reserves the right to cancel the seminar or any workshop before August 29, 2003. Attendees will receive a full refund.
• Transfer: You may transfer from one workshop to another if there is room and you give written notice to the Society Office by on or before September 5, 2003.
• Substitutions: Substitutions will be permitted prior to the start of the seminar, workshop or Conference. No substitutions will be permitted during the course of any of these programs
• No Shows: The attendees who sign up for the seminar, workshop, Conference or combination thereof and do not appear nor send a substitute will NOT receive a refund of their fees.
• Waiting List:If you are on a waiting list for any of the above programs and no space becomes available, you will receive a full refund.
• Membership: To attend the Conference you MUST be a member of the Society. If you are not a member you can submit this application form but your acceptance to attend the conference will be predicated on your becoming a member prior to the start of the Conference. To become a member just complete the Membership Application.
